Certificate Requests
The Vital Records Office of the Will County Health Department maintains records of Births and Deaths that have occurred in Will County.
Records are kept for a period of 10 years only. Records over 10 years old are only available through the County Clerk's Office (815-740-4615) or the State of Illinois (217-782-6553). For
additional information regarding vital records, visit the State of Illinois website at www.idph.state.il.us/vitalrecords/index.htm.
Birth Certificates
Birth certificates can be obtained in person at the Will County Vital Records Office. To request a birth certificate by mail, a photocopy of a valid State issued driver's license
or State ID must be included, plus a list of the following information:
- The child's full legal name
- Date of Birth
- Name of the Hospital or the address where the birth occurred
- Mother's full name, including her maiden last name
- Mother's date of birth
- Father's full name, if it is listed on the record
- Father's date of birth
- Full name of person requesting the record
- Requestor's relationship to the child
- Return address, and daytime phone
- Signature of requestor
- Photocopy of current and valid identification
Payment in the form of money order or personal check, payable to: WILL COUNTY HEALTH DEPT. ( Please note: Out of state checks will
not be accepted, nor will checks that have a different address than either the return address or the identification.)
One certified copy of a birth certificate costs $12.00; additional certified copies of the same record when requested at the same time cost $4.00 each

Death Certificates
Death certificates can be obtained in person at the Will County Vital Records Office. To request a death certificate by mail, a photocopy of a valid State issued driver's license
or State ID must be included, plus a list of the following information:
- Decedent's full name
- Date of Death
- Date of Birth
- Requestor's full name
- Relationship to decedent, or what you need those death certificates for
- Return address, and daytime phone
- Signature of requestor
- Photocopy of current and valid identification
Payment in the form of money order or personal check payable to:
WILL COUNTY HEALTH DEPT. (Please Note: Out of state checks will
not be accepted, nor will checks that have an address that is different from either the return address or the identification.)
One certified copy of a death certificate costs $14.00;
additional certified copies of the same record when requested at the same time cost $6.00 each.
