Pre-Admission Screening
This service is for the use of professional medical/mental health staff only at the facilities listed below. Pre-admission screening for state hospital
admission of severely mental ill adults is mandated by the State of Illinois. Candidates for admission must be subject to involuntary admission,
have a valid petition and certificate for involuntary commitment completed, be medically cleared and have no funding source available (Such as
medical insurance, Medicare or Medicaid). When appropriately requested, mandated 24 hour/day ADULT pre-admission screening services for
state mental health hospital admissions are completed on site by Will County at Silver Cross Hospital and Provena St. Joseph Medical Center.
A petition for involuntary commitment (MHDD 5) may be completed by any adult citizen. This form requests that the person named in the
petition by transported to a mental health facility for a mental health assessment. It does not mandate the transport or assessment. However,
police, fire/rescue or hospital medical personnel may act upon this request. To make the request mandatory a mental health professional who is
qualified or a medical doctor must complete a certificate for examination (MHDD 6).
