Case Management

The people who work in our case management program help our clients to be as independent as possible and to participate more fully in their communities. Our case managers help our clients with a lot of different services.

Some examples are:

finding an affordable place to live,
making appointments to see a medical doctor or a dentist,
getting to your appointments if you have no transportation,
teaching you how to use the public transit system so you can take a bus anytime you're able,
helping you choose foods that are good for you,
helping you to figure out how to do things that you would like to do,
helping you make friends and be active in your daily life,
reminding you to take your medications and teaching you ways to remind yourself.
