POSITION:                   Staff Nurse I, II or III

DIVISION:                     Family Health Services

WAGES:                      Staff Nurse I or II Grade 13   Hourly Wages ($17.27-$29.01)

                                    Staff Nurse   III    Grade 14   Hourly Wages ($19.19-$32.23)

BRIEF

DESCRIPTION:            Provides case management efforts in a variety of settings to clients eligible for Family Health Services.  Assess needs for health and social services; develops an individual care plan in conjunction with client; periodic assessments, follow-up referral activities and asses pregnancy status. 

MINIMUM

QUALIFICATIONS:       Staff Nurse I:  Accredited Associate’s degree in Nursing and be a Registered Nurse by the State of Illinois or reciprocal State required.

Staff Nurse II: Accredited Diploma in Nursing and be a Registered Nurse by the State of Illinois or reciprocal State required.

                                    Staff Nurse III:            Accredited Bachelor’s degree in Nursing and be a Registered Nurse by the State of Illinois or reciprocal State required.

DESIRED:                    Working with pregnant women and Bilingual skills.

REQUIREMENTS:        Personal car in operable condition.  Minimum auto liability insurance.  Verification of U.S. citizenship or employment eligibility.
