Mobilizing for Action through Planning and Partnerships
Background: Launched in 2001 by the CDC & National Association of County & City Health Officials (NACCHO), MAPP is a community wide strategic planning approach to assessing community needs and improving community health. It provides a comprehensive framework for developing and addressing strategic issues of the county. The planning process is driven by a broad based community planning group, called the Steering Committee, resulting in a comprehensive community plan designed to improve the local public health system.
The MAPP process is divided into 6 phases: 1. Organize for Success 2. Visioning 3. Assessments 4. Strategic Issues 5. Goals & Strategies 6. Action Cycle. More information on each of the phases is found on the MAPP Process Poster.
