The following table displays the available vaccines and the cost. Pre-travel consultation is available for $50.00 for the first person; $25.00 for each additional person in the family.

Call 815-740-8143 for and appointment and information.  Daytime, early morning and late afternoon appointments are available.  Please schedule your appointment at least 4 weeks before your date of departure for maximum protection from vaccine-preventable diseases.
