The consumer is presenting with a psychiatric crisis in an ED in the R1SCCS area: Silver Cross Hospitals in New Lenox and Homer Glenn, IL, Presence St. Joseph’s Medical Center in Joliet, IL, Adventist Bolingbrook Hospital in Bolingbrook, IL, Edward Plainfield ER in Plainfield, IL. The consumer is experiencing or presenting with a psychiatric crisis and has a preliminary diagnosis of mental illness or mental illness and substance abuse disorder. The consumer has no insurance (including Medicaid). Consumers must not be acutely intoxicated or delirious as evidenced by a blood alcohol level (>0.08), unstable vital signs, or fluctuating mental status on clinical exam. The ED has completed a medical assessment of the consumer and found and documented on the psychiatric medical clearance form that the consumer does not have a current medical illness or condition that makes the person inappropriate for care in a community residential setting or DHS/DMH Hospital. Evaluators are responsible for completing the EDA from ED’s in the RSCCS area only.
