The referring agency, which is coordinating discharge should provide the following information: consumer’s name, address, and telephone number, discharge date and discharge summary, including medications. The referring agency should also supply the consumer’s demographic and contact information, including the Uniform Screening and Referral Form (USARF), Level Of Care Utilization System (LOCUS), American Society of Addictions Medicine Patient Placement Criteria Second Edition – Revised (ASAM PPC-2R), if applicable, hospital assessment, hospital discharge summary and follow-up instructions including any pertinent medical/lab results
